Scholarship Additional Documents Form

Must be submitted between January 1st and no later than April 15th of graduating year. Documents Required: The following should be submitted with the completed application. • School-produced report showing GPA and grades. • Three signed letter(s) of recommendation from teachers, counselors, instructors, coaches, mentors • A one-page letter describing applicant’s plans for college or trade/technical school and why they should be considered for this award. The letter should give the committee a sense of the passion and level of expertise the student has in their art form or trade and include outside interests or community involvement, as well as details regarding financial need. • If Applicable: One or two PDFs, JPEGs or video links that show samples of the applicant’s creative work.
