Use this form to upload additional supporting documents with the competed application. • School produced report showing class rank, GPA and test scores. • Three signed letter(s) of recommendation from teachers, counselors, instructors, coaches, mentors • A one-page letter describing applicant’s plans for college or trade/technical school and why they feel they should be considered for this award. The letter should give the committee a sense of the passion and level of expertise the student has in their art form or trade and include outside interests or community involvement, as well as details regarding financial need. • One or two PDFs, JPEGs or video links that show samples of the applicant’s work (if applicable).
